Acute Care

This is a branch of secondary health care where a patient receives active but short-term treatment for a severe injury or episode of illness, an urgent medical condition, or during recovery from surgery.

Chronic Disease

An integrated care approach to managing illness which includes screenings, check-ups, monitoring and coordinating treatment, and patient education. It can improve quality of life while reducing your health care costs .

Medication Management

It’s vital that patients follow directions given: the time of the day the medication is taken and how it’s taken. Non-adherence can happen deliberately or unintentionally.

Physical/Annual Exams

This is a routine test your Primary Care Provider (PCP) performs to check your overall health. This can be a doctor, a nurse practitioner, or a physician assistant. You don’t have to be sick to request an exam.

Medical Diagnosis

This is the process of determining which disease or condition explains a person’s symptoms and signs. A history and physical examination of the patient seeking medical care is performed.

Medical Treatment

This is the attempted remediation of a health problem, usually following a medical diagnosis. There are 3 principal types of medical treatments:  Curative, Palliative, andPreventative.

Specialist Referrals

This is a written order from Access Care NM to see a specialist for a specific medical service. Referrals are required by most health insurance companies to ensure that patients are seeing the correct providers for the correct problems.
